Symptom

An error "Failed to find "Certificate Approved" on approval page" and/or "OK; Revocation Failed with error: Failed to parse revocation request" are shown

Cause

You may have multiple Venafi servers running older versions which are pointing to the same database. These attributes changed between versions and these errors usually indicate that they are not in the expected format.

Resolution

Shutdown and upgrade any other Venafi servers which are running on the same database.
